Which IT form should I use?

Is something not working correctly?

Errors, system down, login issues, printer/Wi-Fi problems, Laptop issues etc.

Report an Incident

Use this when something is broken not working right or unavailable.

Is this for a new starter / new user?

New staff member, volunteer, student, locum, etc. who needs account access.

New User Requests

Set up accounts, permissions, email and access for new starters.

Do you need new or replacement kit?

New laptop, monitor, phone, headset, docking station or a replacement device.

Equipment Requests

Order new devices or request a like-for-like replacement should yours be worn out or damaged.

Everything else

Nothing is broken, it’s not for a new user and you don’t need new kit.

Service Request

Use this for changes, advice or help: software installs, access changes, “how do I…?”, relocations, SharePoint/Teams changes and general support.
